When the Mirror Betrays Its Image!
A Case Study at the Limits of Psychotic Diagnosis in the Light of Psychoanalytic Theory

يعالج هذا البحث التحوّل الذي قد يطرأ على علاقة الشخص بجسده وبذاته وبالمكان، عقب حدث جسدي مفاجئ وغير متوقع، ويهدف إلى تحليل حالة سريرية وفق مصفوفة تفسيرية متعددة المستويات (تشخيصي، وتحليلي، وصَدَمي)، ومساءلة العلاقة بين صدمة الإنسان بفقدان جزء من جسده بغياب الموافقة على ذلك، وبين اضطراب صورة الجسد والذات في المرآة. وذلك، من خلال دراسة حالة لشاب خضع لإجراء جراحي تحت التخدير، ثم اكتشف بعد الاستيقاظ أن ختانًا قد أُجري له، إضافة إلى العملية الأخرى التي دخل على أساسها المستشفى، دون أن يكون قد اختاره أو توقعه. ويوضح البحث أن الشاب لم يكن وقتها يفهم كلمة ختان، فأصيب بنوبات من الهستيريا عندما أخبره أهله أن الطبيب اقترح عليهم القيام بذلك تحت الجراحة، ولم يتوقعوا أن يؤذيه هذا الأمر إلى هذه الدرجة.
يطرح البحث الإشكالية الآتية: كيف يمكن فهم التحوّل في علاقة الذات بجسدها وبالمكان عقب حدث جسدي صادم وغير متوقَّع، يخلق شرخًا في صميم الأنا، ويهزّ صورة الجسد في المرآة وفي حدود الأمكنة؟ ويعتمد منهج دراسة حالة نوعية استكشافية بتصميم تحليلي متعدد المنظورات، وذلك لأنه يفصل بين الوصف السريري المباشر، والاستنتاج التشخيصي، والفرضية الدينامية، والتأويل التحليلي.
يلحظ البحث أن هذا الحدث الصدمي أسس اضطرابًا في صورة الشاب في المرآة، وفي حدود المكان، إذ إنه عزل نفسه، وعاش الغضب الشديد والقلق، ثم قام بعمليات تجميلية كثيرة لأعضاء في جسده، دون الحاجة إليها، إلى أن بدأ يفقد الاحساس بحدود الأمكنة والاشخاص بعيدا من أي منطق واقعي، وصار يخاف أن يرتطم به أحد، ثم أصيب جسده في كفّ حركي واضح، مع وساوس ونقص في رغبة الحياة ورهاب اللمس. ويكشف البحث أن هول الصدمة التي تلقاها هذا الشاب، قد دمرت صورة الجسد عنده، وحطّمت مرآة ذاته؛ ففقد الإحساس بألفة الجسد وحدوده المكانية، ولم يستطع أن ينهي الحداد على ما فقده من جسده اليافع في تلك الفترة.

Abstract:
This article examines the transformation that may occur in an individual’s relationship with their body, their sense of self, and their relationship to space following a sudden and unexpected bodily event. It aims to analyze a clinical case through a multidimensional interpretive framework encompassing diagnostic, psychoanalytic, and traumatic perspectives, while questioning the relationship between the trauma of losing a part of one’s body without consent and disturbances in body image and self-image in the mirror.
The case concerns a young man who underwent a surgical procedure under general anesthesia and, upon awakening, discovered that he had undergone circumcision in addition to the other surgical procedure for which he had been admitted to the hospital. This intervention had neither been chosen nor anticipated by him. At the time, he did not understand the meaning of the word “circumcision.” He subsequently experienced hysterical episodes when his family informed him that the physician had suggested performing the circumcision during the surgery. His family had not anticipated that this intervention would affect him to such a profound extent.
This traumatic event gave rise to a disturbance in the young man’s image of himself in the mirror and in his sense of spatial boundaries. He isolated himself and experienced intense anger and anxiety. He subsequently underwent numerous cosmetic procedures on various parts of his body despite having no medical need for them. Over time, he began to lose his sense of the boundaries between spaces and people, in ways detached from ordinary reality testing. He became afraid that someone might collide with him, and subsequently developed a marked motor impairment, accompanied by obsessive thoughts, a diminished desire for life, and touch phobia.
The overwhelming nature of the trauma experienced by this young man profoundly disrupted his body image and shattered his mirror image of the self. He lost his sense of familiarity with his own body and with its spatial boundaries and was unable to complete the mourning process for what he had lost from his youthful body at that time.
How, then, can we understand the transformation in the subject’s relationship to their body and to space following a sudden and unexpected traumatic bodily event—an event that creates a rupture at the very core of the ego and destabilizes the body image in the mirror as well as the boundaries of space?
A qualitative, exploratory case-study methodology was adopted, using a multi-perspectival analytical design. This approach makes it possible to distinguish between direct clinical description, diagnostic inference, dynamic hypothesis, and psychoanalytic interpretation.

* Dr. Sonia Jean Chamoun is a clinical psychologist, psychotherapist, and psychoanalyst, as well as an accredited trainer in psychoanalytic psychotherapy. She holds a PhD in Clinical Psychology from the Lebanese University and currently serves as Head of the Department of Psychology at Branch IV. She is also a member of the Disciplinary Council of the Lebanese Order of Psychologists (LOPSY). She is a member of the World Association of Lacanian Psychoanalysis and the Lacanian Forum of Lebanon and has extensive and rich clinical experience. She is also a founding member of the INSIGHT Center for Training in Psychoanalytic Psychotherapy. Dr. Chamoun represented the Lebanese University on the Colloquium Committee at the Ministry of Education and Higher Education. She also participated in the work of the Ministry of Public Health concerning the review of professional files, following the official enactment of the law regulating the profession of psychology in Lebanon.
